Medical Release & Parental Consent Form - All students who are re-enrolling and have no changes to their medical history or insurance are not required to complete a new medical release. Others should complete a new form for 2024-2025.

Chelsea Tutorials Handbook - please complete the signature page and turn in only the signature page on August 19, 2024. You may print the handbook for your records.

There is a mandatory student/parent/teacher meeting on Monday, August 19th at 9am for last names beginning with A - M and at 11am for last names beginning with N - Z. All elementary students and parents should attend at 9am if at all possible. All paperwork is due at this time. All supply fees, tuition and facility fees are due at this time.
