Medical Release & Parental Consent Form - All students who are re-enrolling and have no changes to their medical history or insurance are not required to complete a new medical release. Others should complete a new form for 2024-2025.

Chelsea Tutorials Handbook - please complete the signature page and turn in only the signature page on August 19, 2024. You may print the handbook for your records.

There is a mandatory student/parent/teacher meeting on Monday, August 19th at 9am for last names beginning with A - M and at 11am for last names beginning with N - Z. All paperwork is due at this time. All supply fees, tuition and facility fees are due at this time.
